Advance Directives can be invaluable tools when planning for the future.

Our Care Manager will review your wishes pertaining to medical decisions:

• artificial nutrition and hydration
• DNR (Do Not Resuscitate Order)
• other difficult medical decisions

We will facilitate a discussion of these important issues in preparation for the completion of:

• a Living Will
• a Health Care Proxy form
• a Power of Attorney
