Coordination of Care

Our Care Manager will provide the following services for the full coordination of medical appointments, including:

• scheduling medical appointments and follow-up visits as prescribed
• arranging transportation to and from the doctor’s office
• hiring a Home Care Aide as an escort
• helping your loved one follow the doctor’s treatment plan

If needed, we will order any surgical or adaptive equipment needed to enhance your love one’s care, including:

• grab bars      

• commode
• wheelchair
• hospital bed
• hoyer lift (helps patient to/from wheelchair and bed)
• personal hygiene items
