Assistance with Medicaid Planning

During the initial consultation and comprehensive evaluation phase, our Care Manager will provide the following services:

• review your entitlement benefits
• assist you to obtain the necessary services
• explain the Medicaid application process to you

At your request, we will also provide you with the following services:

• complete the application for presentation at the Social Service Department of the county where your loved one lives.
• act as your representative during the application process
• complete and submit an application for a pooled income trust
• advocate for you in fair hearing proceedings

We will also work closely with your Primary Care Physician to ensure that you receive the optimal level of home care services covered by Medicaid.

Our Care Managers are highly experienced in obtaining services that are covered by public funds, including:

• Access-A-Ride
• Meals on Wheels
• Personal Emergency Response System (PERS)
• Adult Day Health Care program
