The Patient Review Instrument (PRI) is a required component of relocation to a nursing home facility.
This assessment tool is the most significant component of the decision to accept an individual’s application.
We can arrange for a registered nurse to complete the document called a PRI for you or your loved one to ensure that it reflects your loved one’s functional condition so they can be placed in the facility most suitable for their unique needs.

When a client can no longer manage with home care due to physical and mental decline, the selection and arrangement of alternative housing may be considered, both in New York state and out-of-state, closer to other family members.

• Assisted-living facilities

We help the family to cope with the difficult tasks of negotiating the health care system and provide them with the tools to make the appropriate decisions for them.
