Advocacy and Support

Our care manager will advocate on your behalf to obtain the highest level of services, while in the community, during a hospitalization or when admitted to a short-term rehabilitation center.

We will represent you in various settings to ensure that you receive the appropriate and maximum level of support – including NY State Fair Hearings and Discharge Plan Meetings.
