Your Care Manager will help you do some advance planning that will deter and reduce the risks involved at this important stage of life.

Upon completion of the comprehensive assessment:

• We will recommend how to create a safety network;
• We will customize a list of safety measures for your loved one.

Crisis Intervention

When a crisis arises, our Care Manager will assist and guide you through the situation and facilitate the development of coping mechanisms, including:

• We will maintain a line of communication with the older adult and his/her caregiver, providing instructions and support during and after the crisis;
• We will create and establish emergency procedures and connect the older adult and caregiver with support networks.
